Peptic ulcer called now a days as Acid Peptic Issues(APD) is defined as inflammation of the inner lining of the stomach due to acid peptic digestion. In other words, there is a fight going on in the lining of the stomach against the irritation by H.pylori germs. The characteristic symptom of peptic ulcer is a burning type of pain in the upper abdomen that comes on in an empty stomach.

What causes Peptic Ulcer?

A strong family history of ulcer is found in most persons with ulcer. This family tendency fixes the target organ at the physical level(Annamaya koùa) to manifest the internal imbalance that started in mind(Manomaya and the Práïáyáma Koùa).
All the other causes mentioned in the list are the triggers. They cause an imbalance either by decreasing the protectors or increasing the aggressors. H.Pylori, is a bacterium, which lives on the surface of normal stomach as a harmless opportunist. When the defences of the stomach lining are poor they start penetrating and enjoy the meal from the deeper layers of the stomach.

Dhanurásana swing

Sthiti: Prone Posture
PRACTICE

l Bend the knees and hold the ankles by the palms.
l As you inhale, raise the head and the.chest upwards.Pull the legs outwards and backwards so that the spine is arched back like a bow.
l Stabilize (rest) on the abdomen.
l Do not bend the elbows.
l Look up.
l Keep the toes together.
l Rock the whole body forward and backward rhythmically 5 to 10 times.

How do emotions increase gastritis?

When you get emotionally excited, you trigger off intense activity in the limbic area of the brain. From here you send the information down to hypothalamus which is the controller of the stress orchestra. From here the excitation passes down the sympathetic and parasympathetic nervous system. The parasympathetic (Vagus nerve) stimulates production of hydrochloric acid through release of a local harmone(Gastrin), from the lower end of the stomach (pylorus). The vagus nerve also directly stimulates the acid producing oxyntic cells of the stomach lining. The excitement proceeding down the sympathetic channel narrows down the blood vessels in the lining of the stomach. This results in decreased production of both the quantity and quality of mucus. Due to this decreased blood flow the capacity to repair and restore the damaged lining cells also is slowed down. In addition when you are stressed, it is well known that your immune system becomes less capable of handling the germs around. This inability of the fighter soldier cells (white blood cells) in the lining of the stomach to kill the germs (H. pylori), allows the germs to take the upper hand and penetrate the lining and enjoy their meal from deeper layers of the stomach! Can you see how each one of the ‘Protectors’ is weakened due to the long standing uncontrolled emotions (Stress)? All these happened basically because of your life style of unquenching thirst for more and more of money or name or fame or position. This can further lead to aggressive behavior, drinking, smoking etc., Thus, stressfully demanding situations leading to long standing emotional upsurges cause an imbalance between aggressors and protectors resulting in aggressors taking the upper hand and damage the stomach lining.

HALÁSANA – Páùcimatánásana Swing
Sthiti: Supine Posture
Practice

l Inhale and raise both the legs rapidly by pressing down on the arms.
l Exhale and roll the legs over the head keeping the legs straight and touch the toes to the ground like in Halásana.
l Then inhale and exhale to roll the body rapidly back to starting position.
l Now quickly inhale to sit up.
l Exhale and bend forward into Paschimatánásana keeping the knees perfectly straight. Try to touch the forehead to the knees.
l Rock back and forth alternately into Halásana and Paschimatánásana in this manner 10 to 15 times.

c) Note

l Start slowly and gradually increase the speed.
l The practice should be performed with flowing movements.
l Allow the breathing to be smooth, rhythmic and natural.
Do not bend the knees at any stage of the practice.

Did yogis understand this concept and mechanism of psychosomatic ailments?

You may have noticed that the first thing that an Ayurveda yoga physician would like to correct when you have any chronic illness is the digestive system. He does it by cleansing, balancing and correcting the imbalances. This is because of the knowledge handed down by the ancient yogis who did experiments with their own inner bodily mechanisms through deep meditation and saw that the digestive system is the first target where the stress manifests. The yogis have given us vivid descriptions of how the psychological upsurges result in psychosomatic ailments. This understanding is of practical use for every sufferer of digestive problems, to correct oneself and return to normal health.

GOMUKHÁSANA

Sthiiti: Dandasana.
PRACTICE

l Bend the left leg underneath the right leg so that the heel of the left leg is touching the right buttock.
l Bring the right leg over the top of the bent left leg so that the right heel touches the left buttock.
l Adjust the right knee so that it is above the left knee.
l Place the left arm behind the back and the right arm over the right shoulder.
l The back of the left hand should lie in contact with the spine while the right palm rests against the spine.
l Try to clasp the fingers of both hands behind the back.
l Bring the raised elbow behind the head so that the head presses against the inside of the raised arm.
l Hold the head, neck and spine erect.
l Close the eyes.
l Stay in this position for one minute.
l Unclasp the hands, straighten the legs and repeat with the left knee uppermost and the left arm over the left shoulder.
Mr. S presented himself to us with repeated episodes of peptic ulcer, which was flaring up almost once or twice a year, for 8-10 years. The problem began with pain in the abdomen, acidity, gas, and belching, lasting a few weeks initially and controlled each time by allopathic medication. But, the problem recurred year after year. The year when the ulcer did not trouble him, he would have bowel problem with a lot of flatus, abdominal cramps and a feeling of incomplete emptying of bowel. Colonoscopy would reveal no abnormality and complementary Management would give him relief.
How does yoga understand this? All these physical complaints are at the Annamaya Koùa. Looking at the life style of Mr.S, it was very clear that these periods of bowel upsets were always due to tension both in his business and family.
He said, “Doctor, I know that I am a very sensitive person; I have high ambitions; I am a hard worker (almost a workaholic) and I get irritated if my employees do not come up to my expectations. Being sentimental, when I have emotional conflicts with my wife, parents etc., and because I do not want to hurt them, I keep all my feelings to myself. Of course the business is so demanding that my timings for food and sleep also become erratic. My system has learnt to somehow cope up with all these and be very successful and productive. But there are periods when these situations get beyond my capacity to cope. Then I end up with problems either in the stomach or the bowel which further deteriorates the quality of my life; I get into a viscious cycle, ending up taking medicines for weeks”.

Uddèyána Bandha and Agnisára
Sthiti: Táãásana
Practice

l Stand with the legs 2 to 3 feet apart, bend forwards slightly from the waist and place the palms on the thighs with the arms straight.
l Make yourself quite comfortable in this position.

Stage I: Uddèyána Bandha

l Exhale completely through the
mouth by vigorously contracting the abdominal muscles.
l Simultaneously press the hands against the thighs, tighten the arms, shoulders, neck muscles and lift up the ribs.
l As a result of this, the diaphragm automatically rises up, producing a concave depression of the abdomen. The abdominal wall gets sucked in as if to press the spine.
l Hold this condition as long as comfortable.
l Then inhaling slowly release the abdominal muscles and return to the upright position.
l Rest for a while and then go for the next round.
l Repeat 5 rounds.

Stage II: Agnisára

l In the position of Uddiyána Bandha, move the abdominal wall in and out vigorously like a pump as many times as you can (while holding the breath in exhalation).
l This movement of the abdominal wall is done through mock inhalations & exhalations i.e., it seems as if one is inhaling and exhaling whereas it is not so.
l Then stop the movement of the abdominal wall, release the bandha and while inhaling come up to Táãásana and relax.
l Repeat a few times.

The abdomen is called the magic box of the body. All is well but suddenly like the bolt from the blue, how it may strike a cruel blow which we may not be able to anticipate .One fine day you may experience pain and burning sensation in the stomach which may spread upwards to the throat there may be mild nausea, pain and distension all this may be due to unregulated and excessive production of hydrochloric acid which corrodes the inner lining of stomach and also the lower ends of oesophagus.
This may lead to peptic ulceration the pain and the condition is aggravated by rich spicy diet alcohol and smoking
The medical science and yoga are in accordance in recognizing peptic ulcer as psychosomatic disease. The lining of the stomach is extremely sensitive and is recorder of our emotional status. It blanches during fearful or threatening situation and blushes red when one is in rage and anger. Emotional tension and psychic stress factors are related to the digestive organ and gland via the sympathetic (solar plexus) and parasympathetic (vagus ) nerve path way.
Frequent irritation and stimulation of the mucosal lining results in excess release of hydrochloric acid, which irritates & scars the lining giving rise to auto-digestion which rise to corrosion & peptic ulceration.
Growing worries fear, & constant gastric acid secretion & turning over of the stomach, which causes the damaging effects which decreases the quality of life.
The yogic traditional understanding of the concept & mechanism of psychosomatic ailments is through the 5 layered human existence
The problem of psychosomatic or stress induced Issuess, begin in the manomaya kosa or the mental body. The long standing uncontrolled surges of likes & dislikes which cannot be mended by the directives from vijnanamaya kosa (intellectual level) percolate into the pranamaya kosa (the vital energy body). The prana or energy layer becomes disturbed & violent, resulting in irregularities, manifesting as disturbed functions in the anandamaya or gross body. Some yogic practices which Provides therapeutic relief & cure.

Ramesh was a thin man, easily irritated & restless came arogyadhama, Health Home, in Prashanti Kutiram, Jigni, Bangalore with complaints of upper abdomen pain, bloated feeling& loss of appetite, occasional vomiting. He was diagnosed to have Acid Peptic Issues. He had tried a lot of medications & therapies. He had also tried Ayurvedic medicines & fasting. He hardly found much relief. He was encouraged to go through the Integrated Approach of Yoga Therapy, special techniques for Gastro Intestinal Issuess& especially for . After the first week he started feeling better, his appetite improved & he became more determined. At the end of the 2nd week, he was very confident, his gastric pains had almost ceased. He was off medications and he returned home a happy man, resolving to carry on the yoga practices regularly.

Stand erect with the feet one foot apart and the palms on the hips.

Push the pelvic region slightly forward, exhale and curve the trunk back so that the weight of the body is felt on the things and the toes.

Raise the arms above the head and drop the hands on the floor. Immediately try to strengthen the arms at the elbows and rest the palms on the floor. If the elbows are not stretched immediately as the palms touch the floor, one is likely to bang the head.

After securing the above position stretch the legs and arms straight.

While learning the pose this way it is helpful to use a friend or a wall. Stand about there feet from a wall with your back to it. Curve the back and move the head towards the wall. Raise the arms over the head and rest the palms on the wall. Push the pelvis forward so that the body weight is felt on the thighs and move the palms down the wall until you touch the floor. Use the wall for coming up in a similar manner. After mastering this, only use the wall until you are half-way up. Then learn to do the asana in the middle of the room.

What is irritable bowel syndrome?

It is also called as functional bowel Issues, this is a condition characterized by disturbed motility of the large intestine leading to error of propulsion of its contents. As the word indicates, there is no damage to the intestine and hence is not a killer Issuesbut it could be great nuisance to life. Normally the food that we consume passes through 30 feet length of the small intestine, wherein the digestion and absorption of all nutrients has been completed before it reaches the large intestine. Large intestine has one major function namely, to absorb the valuable water and salts and pass on the faeces into the waste bin, the rectum. From here the expulsion of faeces is subjecte to orders from the higher brain which decides to empty the bowel depending on the time, place and circumstances. A healthy cycle is to empty the bowel once a day early in the morning. Disturbances of this rhythm leads to irregularities of bowel habits termed IBS resulting in either constipation of diarrhea.

What is bowel motility and how is it controlled?

Large intestine has two types of movements :
Type 1: move the contents around within the intestine without moving them forward, known as segmental shuttle contractions. This aids in gaining times and contact with the lining of coon for absorption of the water and salt.
Type 2: a mass movement which squeezes the feaces onwards into the rectum called peristaltic contractions. This is activated early in the morning when you wake up or after breakfast. These movements caused by the contraction and relaxation of the thin layer of muscles in the large intestine are guided by the nerves from brain. The parasympathetic nerves stimulate the peristalsis and hence ensures emptying of the bowel whereas the sympathetic encourages the holding in of the contents. Further, near the terminal part of the rectum, the anus, there are sphincter muscles which are also supplied by nerves from the higher brain i.e., the voluntary nervous system.
Thus, an act of emptying of bowel is guided by increased peristaltic movements of the large intestine, which sends up a message to the brain. The brain (in turn the mind) senses this as an urge to empty the bowel. The mind now decides whether the time and place are congenial (availability of an acceptable clean toilet etc.), sends down the messages via parasympathetic and the voluntary nerves to the annual sphincters (which are normally held constantly contracted) to relax and the bowel is emptied.
The role of psyche is very important even in the normal daily emptying of the bowel. An example of persons like Mahatma Gandhi who could fast up to 21 days and still retain his regular bowel movements through specific exercises called yogasanas. Thus we have enormous capacity to control our basic reflexes. When the mind is disturbed or overworked of speeded up, it leads to erratic functioning of the nervous system. The surges from the emotional cortex working through over stimulations of hypothalamus, passes down the sympathetic and parasympathetic channels similar to what happened in the upper part of digestive system(the stomach). This imbalance manifests as irritability of the bowel.
A regular practice of yoga has helped a large number of subjects to help themselves and be free from their Irritable bowel syndrome.
The asanas used under special techniques for GID either stretch, compress, or twist the abdominal area, so that to bring a awareness to the part, focus and then defocus to give very deep rest to intra-abdominal structures.
Persons with IBS and ulcerative colitis have to avoid many of the brisk loosening practices in standing position and emphasize inverted posture to achieve balance the excess of Apana and Udana.

JATHARA PARIVARTANASANA

Jathara means the stomach, the belly. Parivartana means turning or rolling about, turning around.

Technique

Lie flat on the back on the floor.

Stretch out both arms sideways in line with the shoulders, so that the body resembles a cross.

Exhale; raise both legs together until they are perpendicular to the floor. They should remain poker stiff, so do not bend them at the knees.

Remain in the position for a few breaths. Then exhale, and move both the legs sideways down towards the floor to the left until the toes of the left foot almost touch the finger-tips of the out-stretched left hand. Try and keep the back well on the floor. In the initial stages, the right shoulder will be lifted off the floor. To prevent this ask a friend to press it down, or catch hold of a heavy piece of furniture with the right hand when the legs are turned sideways to the left.

Both legs should go down together, the knees being kept tight throughout. As far as possible keep the lumbar portion of the back on the floor and turn the legs only from the hips. When the legs are near the outstretched left hand, move the abdomen to the right.

Stay in the pose for about 20 seconds, keeping the legs stiff though out. Then move the still stiffened legs slowly back to the perpendicular with an exhalation.

Remain with the legs perpendicular for a few breaths and then repeat the movements by lowering the legs to the right and turning the abdomen to the left.
